A headache rack or cab guard mounts behind the cab to protect the driver from shifting cargo and to provide mounting for lights, chains, and equipment. Installation includes frame mounting rated to resist a load strike.

It is a safety device

A headache rack exists to stop a shifting load entering the cab. That only works if it is mounted to structure capable of resisting the strike, which means the frame or the deck structure rather than sheet metal.

It is also a convenient mounting location for work lights, chain storage, and antennas, which is worth planning during installation rather than drilling later.

Yes. Custom fabrication is often the right answer when the cargo profile or the accessory requirements do not match a stock unit.
Does it need to attach to the frame?
To do its job in a load strike, yes. Mounting to deck sheet alone will not resist a real impact.
Can lights be integrated?
Yes, and doing it during installation is much cleaner than adding them later.
